The video is divided in four sections: 1. Design of the portal. 2. Anatomical study that confirms its safety. 3. Performance in real practice. 4. Clinical utilities of the portal.
The design of the portal follows three rules: • One single incision should allow the access to both the radiocarpal and midcarpal joint. • The portal should be centered into the lunate. In this way the hole joints would be easily inspected and the capsule would be broken through its less resistant portion. • The volar structures at risk should be protected not only by retractors, but also by tendons, to minimize its injury.
The safety of the portal has been confirmed after the anatomical study, as neither was there damage to any structures at risk or to the flexor tendons.
The majority of arthroscopies can be performed through dorsal portals, and hence volar central portals are not for a routine use. But it can be useful in some techniques, especially in the advanced ones and we trust that it use can facilitate the development of new arthroscopic techniques
