Christina offers a mini-promotional presentation that describes a model of care delivered through a novel tool built by clinicians over a decade of research at Wake Forest University in North Carolina. Christina co-developed and led the "Transition Stroke Program" and served as Lead APP in implementation of the "Compass Trial". Christina and her team are currently in discussion with several leading academic health systems to implement this tool for management of their post-acute patients.
