read more ↘ cause instability of the extensor pollicis brevis (EPB) and abductor pollicis longus (APL) tendons in zone 7 of the extensors. This can elicit major pain and dissability.
Method: We would like to present a stabilization technique, using an autologous graft harvested from the extensor retinaculum of the ipsilateral side. Advantages of this technique, as well as pearls and pitfalls are thoroughly described.
Results: The intraoperative tests show a clear dislocation of the tendons out of their sulcus on preoperative ultrasound examination, as well as intra-operatively. The reconstructive measure results in stabilization of the tendons upon flexion of the wrist and abduction of the thumb.
Discussion: The described technique offers an immediate and durable stabilization of the tendons. This is confirmed in our series, published in the J Wrist Surg. Another key message of this video is that a systematic reconstruction of the retinaculum in primary De Quervain surgery, prevents this painful complication.
