We present a novel treatment for severe osteochondral lesions that do not improve with conservative treatment. It is a chondrocyte autograft system plus plasma rich in platelets and thrombin (Autocart .Arthrex system) whose main advantage is that it is performed in a single act, of moderate technical difficulty (open or arthroscopic) and with a moderate economic cost.
